There will be a plan to accommodate and ensure the safety of children who have food allergies and other dietary restrictions. If a child requires a special diet due to a medical reason, the staff member will provide the "Food Substitution Physician's Statement". They will assure that the required medical authority's signature is collected.
The medical statement must specify:
- The participant's disability or medical condition
- The foods to be omitted
- Suggested substitutions and or accommodations
The medical statement must be signed by:
- A physician for participants with a disability
- A physician, physician's assistant or registered nurse for non-disabled participant.
